Wormholes have a serious inconsistency issue, a wormhole will spawn, but only specific people can see and use it. For example, I have a station with a fitted wormhole drive, I fire up the WH and I see nothing but the flash, my friend sees and uses the wormhole but I pass right through the empty space. I can see the destination, and even the line to the origin point, but I cannot enter the wormhole. This happens to many people, and it is not always the person spawning the wormhole that has the issue. I should also note that this seems to happen more frequently at times of high latency.
My suggestion is to make wormholes part of the replication system so that they can be consistent between all clients.
Please confirm this bug and give any more information that you have...

08:37:17 [:SA:]Assault Ship Basroil: "Also, note, WH's often get bugged when you try to WH from, or into, a dictored area."

I found this old video along with two others demonstrating the bug MANY versions ago... This bug is still around and acts just as it did then. The video shows another player using the wormhole just fine, while it is useless to my client. The bug is as old as 1.517 (which is around the time the source code for Medusa was released)
Here are the other videos:

yes, if you wh from a enemy dicto, your jd never becomes available again.
(happen to me 3 times about a month ago so i quit using them)

Jumpgates do this too, also if the Interdictor is the one that goes through the WH/gate.

I believe I found a pattern - either that or a very, very convenient coincidence.
Hopefully this doesn't get difficult to understand...
From 4.6.-3.7 in R33 to Mycopia in Luyten, I predicted 10 times in a row if the WH would be visable to me or not.
If I moved ahead just the tinyest bit (<5gu) then stoped before the WH device finished charging, and then used the WH device, the destination end would appear, but not my end.
If I didn't move at all between each attempt then the WH spawned 100% of the time I tried.
If I move a larger distance (100gu or so) and then stoped before the WH device was charged, the WH is created.
Is it possiable that the server asks for the players position when the WH is created, and if the players position is not in the same location that the server was expecting the player to be in then the enterence is not created, due to the server being confused in not knowing where to make the enterance? I say this thinking that maybe if I only moved a small amount the server would not be informed of my moving until I attempted to create a WH, and therefore have old data?
Testing Conditions:
I doubt most of these things would make a difference, but just in case:
- UGTO Command Dread with WH Device.
- High latency (varied from 600-800)
- Was running 1x ecm.
- Supply Plat directly infront of me between me and where the WH would spawn
- Supply plat was connected to my ship via tractor beam
- Gaifen ~2.5k away from me orbiting Fury the whole time.
Also, I noticed a few times that when I pushed a plat through the WH infront of me with my tractor beam, my energy would be put to 0 as soon as the connection to the plat was broken.
